URGENT MESSAGE:

08/24/22

Dear Parent/Guardian of an incoming 7th or 8th graders:

Be sure your student has shown proof of their Tdap or Varicella vaccinations. Per state law, without proof of these vaccinations, your student may be excluded from school.

Please contact your doctor and make sure your student is up-to-date on ALL school-required vaccinations. If your student received their shot(s) over the summer, please bring your student's vaccination record to our school office before August 31, 2022.

If you feel your student is exempt from these vaccination rules, please contact Nurse Gibson immediately. 

If you do not have insurance or your student’s primary doctor is unavailable, please note the Long Beach Health Department will be providing vaccinations the week of August 30th. See letter below for details.

MEDICATION AT SCHOOL

Does your student need an inhaler or other medication while on campus? Please note, students are NOT allowed to carry medication unless certain forms are completed EACH school year. This paperwork makes sure our school nurse is aware of your student's health issues and can quickly help if needed. 

If you want your student to carry their medication, please complete this form:

Permission to Carry Medications at School: ENGLISH/SPANISH

If you want your student's medication kept in the Nurse's office, complete one of these forms:

Medications at School - Parent Request (SPANISH)

PLEASE NOTE - Medication paperwork will not be accepted by the front office unless it is...

  1. Turned in by an adult

  2. Completely filled out with a signature from a guardian AND signed & stamped by healthcare provider 

  3. Turned in WITH the ordered medication


CHANGES IN YOUR STUDENT'S HEALTH

Has your student's health changed since last school year? Please contact the school nurse if your student has... 

  • Had a major illness, major accident or hospitalization in the past 12 months

  • Received a NEW medical diagnosis

  • Started NEW medication 

If your student has had a significant change in their health, please complete and turn in a new Permanent Health History (ENGLISH/SPANISH)
